Model Release for
Gordon Denman and Images by Gordon
I,
__________________________________________________ (Please Print) for good
and valuable consideration, the receipt of which is acknowledged by terms
listed below, give Gordon Denman (Photographer) permission to photograph
me and use the media to his best choice and decision of use for any means
he deems necessary, this to include any media form he deems acceptable
without any verbal and/or written consultation/agreement by me about said
media.
Furthermore, I authorize
Gordon Denman to use this media for any and all purposes deemed necessary
by him in the course of sales and promotion, personal, business and
internet promotional needs, and allow and authorize Gordon Denman to act
on his best determination as the sole agent for the sale/use of this
imagery - I do also forgo receiving any monetary compensation from sales
of other photographs and media that are not of me or under my control
resulting from the use of my media and images.
I hereby grant and
authorize Gordon Denman the absolute right and permission to copyright
and/or publish the works in which I am included, in whole or in part, in
any media form chosen by Gordon Denman. I also grant and authorize Gordon
Denman to sell, publish distribute and display images of me in any media
form – print, DVD and Compact Disk, website or publication and may use the
images of me for art, advertising, trade or any other lawful purpose
whatsoever, without any further compensation of any nature whatsoever.
.I also agree to not hold
liable Gordon Denman, or any authorized agent, employee, assistant, client
or purchaser of media and content from Gordon Denman, for any loss or
injury sustained before, during or after the completion of this or any
photography session, photography shoot, meeting or travel, in any part or
whole of either a person or material nature in any and all forms.
Also, any and all restrictions and/or exemptions agreed upon between
Gordon Denman and myself for the use and showing of any media resulting
from any and all photography sessions shall be listed below and agreed
upon by initialing this release in the space provided.
This release will be held
enforceable from date(s) listed below and agreed to as legal agreement
between all parties until notified of change by either or any of the
participating parties, from the date(s) listed below until such
notification, also covering any and all future works, photography sessions
and meetings or events. Photo Id provided.
All advance modeling,
shooting and production fees and expenses are recoverable from the sales
of the imagery.
I agree to all of the
above and have signed this release stating that I have been placed under
no duress to do so.
